Assist with mobility and perform functional nursing care based on care plans, including transfers and usage of mobility aids. Build professional relationships with residents, family members, and colleagues for effective communication and support. Assist with assessments and procedures as per the State Nursing Practice Act, including vital signs and skin care. Adhere to Infection Control guidelines to ensure a safe environment. Support care protocols for falls prevention, safety, and nutritional needs. Work collaboratively with Nurse Leaders to respond to resident needs and report any changes in status. Support residents in participating in social and recreational activities. Document care provided accurately as per individual Care Plans.
