As a Medical Social Worker , you will play a vital role in providing personalized social and emotional support to patients recovering in the comfort of their homes. Collaborating with an interdisciplinary care team, you will assess patients' social needs, coordinate community resources, and contribute to their overall well-being.

As a Medical Social Worker, you will:

• Assess patients’ social and emotional states related to their illness or injury.
• Evaluate the interplay between medical needs, financial resources, and community support.
• Identify and secure community resources to aid in patient recovery.
• Guide patients and families in coping with social and emotional challenges.
• Collaborate with physicians and health team members to address significant social factors affecting health.
• Monitor and report changes in patients’ conditions to the Clinical Manager.
• Participate in discharge planning and care coordination activities.
• Supervise and mentor Social Work Assistants to ensure compliance with standards.
• Maintain thorough documentation of patient interactions and care plans.

Required Skills and Experience:

• Master's or doctoral degree in social work from an accredited institution.
• Valid social worker licensure in your state of practice.
• Reliable transportation, valid driver’s license, and auto insurance.
• Current CPR certification.
• A minimum of one year of experience in a healthcare setting, preferably in home health or hospice.
• Strong knowledge of discharge planning and community resources.
• Excellent communication and interpersonal skills.

This position offers a flexible schedule, allowing you to balance your professional and personal life while making a meaningful impact in the lives of others.

The pay range for this role is $59,300 - $80,900 per year, depending on experience and location. We also offer a comprehensive benefits package that includes:
