Description

We are looking for a compassionate and organized Hospice Transition Care Coordinator to join our dedicated team. The ideal candidate will be responsible for coordinating and managing hospice care services for patients and their families. This role requires a deep understanding of hospice, excellent communication skills, and the ability to work collaboratively with a multidisciplinary team. The Hospice Transition Care Coordinator will act as a liaison between patients, families, healthcare providers, and other support services to ensure that each patient receives comprehensive and personalized care. The successful candidate will have a strong background in healthcare, with experience in hospice or palliative care, and a commitment to helping families to get the services they need. Responsibilities

  • Identify and assess patients for needed services, including medical, emotional, and spiritual support.
  • Act as a liaison between patients, families, healthcare providers, and support services.
  • Ensure compliance with all regulatory requirements and standards of care.
  • Provide education and support to patients and families regarding hospice care and services.
  • Facilitate communication and collaboration among the multidisciplinary care team.
  • Maintain accurate and up-to-date patient records and documentation.
  • Conduct regular assessments to identify patient needs and preferences.
  • Coordinate and schedule home visits, medical records requests, and other services.
  • Provide crisis intervention and support during emergencies.
  • Participate in team meetings and case conferences to discuss patient care.
  • Provide training and support to hospice staff and volunteers.
  • Participate in quality improvement initiatives and projects.
  • Manage and resolve patient and family complaints and concerns.
  • Stay current with best practices and advancements in hospice care.

Requirements

  • Active LVN license
  • Minimum of 2 years of experience in hospice or palliative care.
  • Strong understanding of hospice care principles and practices.
  • Excellent communication and interpersonal skills.
  • Ability to work collaboratively with all types of patients and families.
  • Ability to adapt as needed to different environments.
  • Strong organizational and time management skills.
  • Compassionate and empathetic approach to patient care.
  • Ability to handle sensitive and confidential information.
  • Proficiency in electronic medical records (EMR) systems.
  • Knowledge of regulatory requirements and standards of care.
  • Ability to provide emotional support to patients and families.
  • Strong problem-solving and critical thinking skills.
  • Ability to work independently and manage multiple priorities.
  • Flexibility to work evenings, weekends, and on-call as needed.
  • Valid driver's license and reliable transportation.
  • CPR certification preferred.
  • Experience with crisis intervention and conflict resolution.
  • Commitment to ongoing professional development and education.
  • The ability to speak a second language is a plus.
