Identify and address strengths and barriers that could impede the development of skills needed for independent functioning in the community. Support the individual and the family in crisis situations; provide personalized interventions to develop, facilitate, or enhance an individual’s ability to make informed and independent decisions. Patient care may include children at risk, adults with severe mental illness, individuals with chronic substance abuse, co‑occurring disorders. Ensure adherence to hospital and departmental policies and procedures. Patient care assignment may include neonate, pediatric, adolescent, adult, and geriatric age groups. Detailed Responsibilities PATIENT CENTERED MED - Adhere to and promote core expectations of the Patient Centered Medical Home or Patient Centered Specialty Practice. NAVIGATION/ PATIENT ADVOCACY - Work to eliminate barriers to access to culturally competent medical services for all patients; assist patients to access community resources and financial assistance. COLLABORATION - Collaborate with medical providers, multidisciplinary team members, and community resource providers to assist patients to meet treatment goals. LIFE SKILLS - Facilitate and encourage development of skills in ADLs, interpersonal coping, socialization, community functioning; adapt home, school, work, recreation, and social environments; promote self‑care, nutrition, money management. DEVELOPMENT - Plan for professional growth related to goals; acquire knowledge and experiences to meet goals. WELLNESS - Offer information and resources about mental condition; serve as mentor to promote recovery and resiliency; teach symptom monitoring, management, and relapse prevention skills. WELLNESS - Provide information about illnesses; assist with medication knowledge, side effects, and discuss concerns; facilitate self‑motivational skills for medication regimen. SERVICE PLANS - Assist in development and coordination of individual’s service plan based on strengths and goals, including crisis and advanced directives. RESOURCE COORD - Assist client in obtaining access to and coordination of rehabilitative, medical services, financial benefits; support safe affordable housing; support individuals in the Judicial System. SUPPORT SERVICES - Encourage development and succession of natural supports in workplace, school, family, and community. ASSESSMENT - Identify personal strengths, needs, barriers; conduct ongoing assessments to measure effectiveness of services. CRISIS INTERVENTION - Assess, support, and intervene in crisis; develop and use crisis management plans; encourage alternatives to ED and inpatient services. TRACKING - Coordinate and monitor use of services; maintain contact with external providers; conduct post‑discharge follow‑up. DOCUMENTATION - Document client encounters; submit billing documentation; maintain comprehensive client files. STATISTICS - Maintain and report statistics regarding programs and services. DEVELOPMENT - Enhance professional growth through education, literature, in‑services, meetings, workshops. PATIENT SAFETY 1 - Follow patient safety policies and procedures. PATIENT SAFETY 2 - Seek opportunities to improve patient safety. PATIENT SAFETY 3 - Identify and correct environmental conditions that may put patients at risk.
