Summary:

• This role involves providing health coaching and case management services across the health continuum.
• The position is remote, based in Philadelphia, PA, and is a contract opportunity.
• The Care Manager will engage with members to support self-management and coordinate care effectively.

Requirements:

• Current, active, and unrestricted Pennsylvania Registered Nurse licensure.
• Three years of clinical, case management, and/or disease/condition management experience.
• Exceptional communication and problem-solving skills.
• Ability to work flexible hours.
• Basic computer skills.

Key Responsibilities:

• Function as a primary nurse for members across various health services.
• Engage with members to develop and support self-management abilities.
• Provide clinical coaching support for acute symptoms and chronic conditions.
• Conduct assessments to identify needs and develop care plans.
• Facilitate communication between patients, families, and healthcare teams.
• Identify resources to assist members in achieving health goals.
• Ensure compliance with regulatory requirements and accreditation standards.

Qualifications:

• Must be PA, NJ or DE State Licensed Registered Nurse
• Current, active, and unrestricted Pennsylvania Registered Nurse licensure.
• Three years of clinical, case management, and/or disease/condition management experience.
